New Employee Orientation
Welcome to Central Management Services
In order to streamline our New Employee Orientation, we have included the forms below that will need to be completed to begin your employment with us. Whether you have worked for another state agency or this is your first time, you will need to complete all the required documents in the first section. If you have any problems completing any of the forms or do not have the means to do so, please contact us at 217.785.3278 to make other arrangements.
A few items to note:
- When filling out these forms, if you are asked for your work email address or work phone and you are not certain as to what it is, please put your personal contact information.
- When asked for your agency or bureau, indicate CMS.
- When asked for Pay Code, leave that field blank.
- PLEASE REMEMBER TO SUBMIT 2 FORMS OF ID, TYPICALLY A DRIVERS LICENSE AND SOCIAL SECURITY CARD OR PASSPORT CARD AS WELL AS A VOIDED CHECK FOR PAYROLL PURPOSES.
- In order for the forms to work properly, please use Internet Explorer to access them.
Required Forms - Please print, complete and bring the following forms to Orientation:
- W-4 Federal Withholding – Page 1 is the only requirement
- W-4 Illinois Withholding – Page 2 is the only requirement
Form I-9: Employment Eligibility Verification- If you have problems accessing the above, print and complete this version.
- Please complete Section 1 Employee Information and Attestation on page 1. CMS will complete Section 2 Employer Review and Verification.
- Direct Deposit Authorization
- Benefits Transfer Form (current state employees only)
- New Hire Tier Determination
- Ethnic Origin Survey
- State Property Form
- Certification of License and Auto
Optional Forms to Bring to Orientation:
- Payroll Deduction Authorization
- State Service History - Complete this form ONLY if you have worked for the State previously AND your return to CMS is your first day back after a break of 4 or more days.

- Health, Dental & Life Choices (new employees only) - due within 30 days of hire
- Register for eTime – CMS Time and Attendance Application
- Register for EPASS – Pay Stub Information System
